The solution is to go into Dynamics GP > User Preferences and set the Window Command Display to Menu Bar, then close and reopen Dynamics.
This will change the new command bar back to its old look and allow you to add the buttons to VBA. The bar will change from this:
to this:
You can put it back after you've added the button to VBA.
