This seems to be a bug and can be resolved one of two ways.
If you switch between companies using the bottom left corner, this will trigger the duplication of lines in the Additional menu.
Solution #1: You will need to log out of GP fully and log back in to switch companies.
Solution #2: According to eOne, this has been fixed in the latest 2013 & 2015 releases of Extender.
